Say Boo! to the Flu: 2018 Application

Basic Information

Thank you for your interest in participating in the 2018 VNAA-Clorox "Say Boo! to the Flu!" program. This application must be completed in its entirety to apply for grant funding and/or material support. Please provide as much detail as possible so we can understand your intended clinic(s) and the audience you serve.

You must answer ALL questions for your application to be considered for grant funding.

1. Do you plan to host at least one flu clinic in September/October 2018, focused on immunizing children under the age of 18 and their families? *This question is required.
